Center for Human Development – LaGrande, OR
To enhance the success of medical/behavioral care plans by providing one–time funding for goods and services necessary to the success of the care plan.  Proposed purchases include:  dumpster/garbage services, cribs, windows, household safety items, cleaning supplies, among other items.

Community Action Resource Enterprises (CARE) – Tillamook, OR
To prevent negative health outcomes by providing resources that allow high–risk families, children and seniors to be proactive about their healthcare.  Proposed purchases include: diapers, formula, children’s Tylenol, baby food, rent assistance and medical supplies, among other items.

Coos County Public Health – Coos Bay, OR
To improve the health of individual clients by meeting an urgent need with a low cost solution.  Proposed purchases include: taxi vouchers, kitchen equipment, medical lab tests, among other items. 

Deschutes County Health Department – Bend, OR
To assist in meeting client needs to reduce transportation, child development & chronic disease management barriers. Proposed purchases include: gas cards, bus vouchers, (used) clothing and tuition for self-care classes.

Ecumenical Ministries of Oregon – HIV Day Center – Portland, OR
To provide bus passes/tickets, hygiene supplies and nutritional supplements to low income, HIV positive Day Center clients. 

Insights Teen Parent Program – Portland, OR
To improve the health of teen parent families by providing access to funds for urgent, health–related needs and personal products that are not covered by the Oregon Health Plan.  Proposed purchases include:  hygiene supplies, child safety items, OTC medications and vital record documents.

Josephine County Public Health – Grants Pass, OR
To allow expeditious response to emergent problems as they present in client population to better support the health and well being of the community.

Multnomah County Health Dept – TB – Portland, OR
To support low income patients in completing their TB treatment by meeting their urgent/unusual health related needs when no other resources are available helping to decrease the spread of TB in the community.  Proposed purchases include:  energy assistance, nutritional supplements, housing assistance, transportation vouchers and housing assistance.

Polk County Public Health – Dallas, OR
To provide fast, flexible health-related services to or supplies in a timely manner to clients with a special need that cannot be met through other resources.  Proposed purchases include:  medical support items, food and utilities, among other items

Portland Impact – Portland, OR
To support the overall health, comfort and self–sufficiency of low–income, senior and disabled adult clients by providing for their one–time health needs with immediacy and flexibility.  Proposed purchases include: wheelchairs, walkers, canes, glasses and magnifiers, among other items.

Virginia Garcia Memorial Health Center – Washington County
To provide transportation to migrant and seasonal farm–workers and other low–income clients for medical appointments with specialists & primary care physicians.

Washington County Oregon – Department of Health & Human Services – Hillsboro, OR
To provide field team clients with good and services needed to improve or maintain their health and development.  Proposed purchases include:  car seats and high chairs, bus tickets, breast feeding supplies, among other items.

Yamhill County Health Department – McMinnville, OR
To provide health–related resources to clients not available elsewhere to enhance family functioning, positive parent–child interaction and alleviate some of the emotional stress incurred from financial hardship.  Proposed purchases include:  infant supplies, phone cards, childcare and breastpumps among other items.
